Updated the installation/troubleshooting instructions and removed the duplicate 'turbolinks' referenced in the Gemfile

Removed DCC specific README.rdoc and then revised/consolidated the text into the README.md file. Added a line to .gitignore to prevent README.rdoc from coming through again. This will allow local forks of the repo to maintain their own README
