Some CSs improvements

Dynamic form should load & unload when a question is opened or closed

Redesigned the interface for better flow when editing the plan content

Example answer area should be displayed for structured questions

Table header should be displayed is locale is not present

Table header is now displayed for sub fragment list, when option is available in the template

User have now to click an 'Answer' button before answering a question for the first time

Added support for 'const' property in templates
